C3: Care Coordination Centre

Nova Scotia Health’s Care Coordination Centre (C3) improves how we coordinate the movement of patients throughout their healthcare journey. It provides a real-time view of access to care, patient flow, and health system resources to support decision-making. It also generates actionable insights to drive continuous improvements in delivering healthcare and improving the quality of patient care.

We have implemented a phased approach to opening Care Coordination Centres across the province. The first is located at the Halifax Infirmary site of the QEII Health Sciences Centre with centres opening in all Nova Scotia Health zones in early 2024. These centres support teams to identify issues and find solutions together, using real-time data. The result is improved patient access and flow, more efficient bed management and discharge planning, better scheduling of staff, and more collaboration within and between health zones across the province.
